Mandatory Vaccinations

Depending on your test results, age, and medical history, the following vaccines are mandatory: 

• Measles, Mumps & Rubella (MMR) 

• Varicella (Only for those who did not have chickenpox as a child and no documentation of 2 doses of the varicella vaccine)
• Influenza (Per the CDC (Centers for Disease Control), Flu season from 10/31 – 3/31 each year) 
• COVID-19 vaccine- 1 or 2 doses (J+J is 1 dose) 
• Hepatitis B vaccine (only the first dose needed for the immigration. It is advised that you get the second dose in 1 month and third dose in 6 months) 
• TDap vaccine (Tetanus) done every 10 years 

• Pneumococcal vaccine (required for ages above 65 yrs )

Procedure for those with Valid Insurance

1. We review your records to determine if you need vaccinations. If you do our civil surgeon will write you scripts for the vaccination required.  

2. We can either draw your labs and take your urine sample here or give you a lab slip to get the drawn directly at Quest Diagnostics or Lab Corp. To save time, you can also get these labs done at your primary care and simply bring the original copy of lab results. Below are the three tests required. 

These are:  

1. Quantiferon Blood Test: a screening test for Tuberculosis that is mandated for all persons over the age of 2 years old. 

2. RPR Blood Test (15 yrs and older): a screening test for Syphilis.

3. Gonorrhoea Urine Test (15 yrs and older): a screening test for Gonorrhoea  

If you do not have your vaccine records, no problem. You can choose to get the vaccines or be tested for vaccine immunity by having a blood test called an “immune titer”.

Procedure for those WITHOUT Insurance

You may choose to get the blood draw and vaccine at our facility. NOTE the Varicella and Pneumococcal vaccines are NOT available at our facility. If you need varicella and/or pneumococcal vaccine, we advise you to get the vaccine at the pharmacy (Our Medical Director will write you a script for that). The other vaccines can be administered at our office on the same day as the varicella and/or pneumococcal vaccine injection. If you do not get the remaining vaccines at the same time as the varicella and/or pneumococcal, then you will need to wait three weeks to get the remaining vaccines. 

Next, your Civil Surgeon will conduct a physical examination, review your test results, and complete all paperwork relating to your medical exam, including form I-693.You will receive a sealed copy of your I-693 federal form, which you will send to USCIS and a separate copy for your records.
